Is Tirzepatide FDA-Approved in the Philippines? 2026 Status

TL;DR

Tirzepatide FDA approval exists at two levels and it is worth keeping them apart. In the United States, the FDA approved tirzepatide as Mounjaro for type 2 diabetes in May 2022 and as Zepbound for chronic weight management in November 2023. In the Philippines, Mounjaro is registered with the Philippine FDA and dispensed by prescription through a small number of pharmacies. Research-grade tirzepatide vials, including those Noki Labs supplies, are not an approved medicine in either country; they are sold as research-grade peptides for personal wellness with a laboratory certificate rather than a drug registration.

Tirzepatide FDA approval in the United States

Eli Lilly developed tirzepatide and holds the US approvals. The timeline that matters:

Date Brand Approved use
May 2022 Mounjaro Glycaemic control in adults with type 2 diabetes, alongside diet and exercise
November 2023 Zepbound Chronic weight management in adults with obesity, or overweight with a weight-related condition
December 2024 Zepbound Moderate to severe obstructive sleep apnoea in adults with obesity

Both brands are the same molecule at the same strengths, 2.5 mg through 15 mg weekly, in pre-filled pens and, since 2024, single-dose vials sold through Lilly's own channel. The approvals rest on the SURPASS diabetes programme and the SURMOUNT obesity programme. SURMOUNT-1 (NEJM 2022) supplied the headline weight number, 20.9 percent mean loss at 15 mg over 72 weeks in 2,539 adults, and SURPASS-2 (NEJM 2021) supplied the diabetes comparison against semaglutide 1 mg.

When someone asks "is tirzepatide FDA approved", the accurate answer is that these branded products are approved. The FDA approves specific products made by specific manufacturers under specific quality controls, not a molecule in the abstract.

Philippine FDA status as of September 2026

The Philippine Food and Drug Administration issues a Certificate of Product Registration (CPR) to each medicine that passes its review, and only a company holding a License to Operate as a drug distributor may import and sell it. Mounjaro has a CPR and is sold by prescription. Local press in 2026 reported pharmacy prices of roughly PHP 5,000 to 6,000 per pen at the lowest strength and considerably more at higher strengths, with stock harder to find than semaglutide; treat those as market observations. Zepbound is not marketed under that name in the Philippines.

Two practical consequences follow. A branded tirzepatide pen is available only with a prescription from a Philippine-licensed physician, at a licensed pharmacy, and is not covered by PhilHealth. And any registration can be checked yourself: the FDA's online verification portal lists every current CPR by brand name, generic name and registration number, which is the quickest way to confirm what a seller is claiming.

Off-label prescribing

The local registration follows the diabetes indication. Philippine physicians may prescribe a registered medicine off-label for weight management, and many endocrinology and bariatric clinics in Makati, Taguig, Cebu and Davao do. That is a matter of clinical judgment, not a separate FDA approval.

What "research-grade" means and why FDA approval tirzepatide questions get confused

Almost all tirzepatide sold online in the Philippines, including Noki Labs' vials, is research-grade. The term describes a synthesised peptide supplied as a lyophilised powder with an independent laboratory certificate of analysis showing identity and purity, but without a drug registration, without a manufacturer's finished-product licence, and without the sterility and stability certification that a registered medicine carries.

The distinction matters in both directions. It means the research vial has no regulatory approval to lean on, so the buyer is relying on the lab report and the supplier's handling. It also means that a seller who describes a research vial as "FDA approved" is misleading you, because the approval belongs to Mounjaro and Zepbound, not to the powder in the vial. Noki Labs supplies research-grade tirzepatide for personal wellness use, and we publish the certificate for every batch on our lab testing page so that the claim we do make, tested purity, can be checked.

Where research-grade peptides sit under Philippine law, including the Bureau of Customs angle, is covered in our companion explainer on whether peptides are legal in the Philippines.

Is tirzepatide safe: what the approval record says

An approval is, in part, a safety verdict on the branded product in the studied population. The trial record behind it is the best evidence available on whether tirzepatide is safe, with the caveat that it describes a pharmaceutical product used under supervision.

Common effects

Nausea, diarrhoea, reduced appetite, constipation, vomiting and indigestion were the most frequent adverse events in SURMOUNT-1, concentrated during dose escalation and mostly mild to moderate. Discontinuation because of adverse events was higher than placebo in every tirzepatide arm but remained a minority of participants. Injection-site reactions and fatigue were also reported.

Serious cautions carried on the approved label

  • Thyroid C-cell tumours. Seen in rodent studies, unproven in humans; the label excludes anyone with a personal or family history of medullary thyroid carcinoma or MEN2.
  • Pancreatitis. Reported in trials; stop and seek care for severe persistent abdominal pain.
  • Gallbladder disease. Rapid weight loss raises gallstone risk on its own, and the class adds to it.
  • Low blood sugar when combined with insulin or sulfonylureas.
  • Kidney injury secondary to dehydration from vomiting or diarrhoea, a real concern in Philippine heat.
  • Delayed gastric emptying can change how oral medicines, including oral contraceptives, are absorbed.
  • Pregnancy. Not to be used while pregnant or trying to conceive.

Our tirzepatide side effects guide goes through each of these with the management steps that trial protocols used.

Warnings from Philippine bodies about unregistered tirzepatide

The Philippine College of Endocrinology, Diabetes and Metabolism issued a public warning in 2026 about compounded semaglutide and tirzepatide products circulating online, noting that their safety, potency and quality cannot be confirmed. The FDA itself periodically publishes advisories naming specific unregistered products that are being marketed with therapeutic claims. Both warnings are aimed at a real problem: vials with no lab report, pens with no box or registration number, and sellers promising outcomes.

The response Noki Labs takes is transparency rather than argument. We do not describe our vials as a medicine, we do not claim they treat any condition, we publish batch testing, and we tell every buyer to work with a licensed physician. If a seller does none of those things, the warnings apply to them. Our guide to spotting fake peptides in the Philippines lists the checks.

How to verify what you are buying

Tirzepatide FDA approval claims are easy to test, because approvals leave a paper trail and research vials leave a lab trail.

  1. Branded pen: look up the registration number on the FDA verification portal, buy only from a licensed pharmacy, and expect to show a prescription.
  2. Research-grade vial: ask for the batch certificate of analysis, check that the lab is independent and the batch number matches the label, and confirm the seller ships cold-chain.
  3. Either: be wary of any listing that uses the words "FDA approved" without a registration number to back it up, or that quotes a price far below the market.

FAQ

Is tirzepatide FDA approved?

The branded products Mounjaro and Zepbound are approved by the US FDA, for type 2 diabetes and for chronic weight management respectively. Research-grade tirzepatide vials are not approved products.

Does tirzepatide FDA approval apply in the Philippines?

US approval does not transfer. The Philippine FDA separately registered Mounjaro, which is sold locally by prescription. Check the registration on the Philippine FDA verification portal rather than relying on a seller's claim.

Is tirzepatide safe to use?

In trials the branded product was generally well tolerated under supervision, with GI effects most common and a short list of serious cautions. Research-grade vials carry the added variable of unverified manufacturing, which is why the lab certificate and physician oversight matter.

Is Noki Labs tirzepatide FDA approved?

No. Noki Labs supplies research-grade tirzepatide for personal wellness use, third-party tested per batch. We do not sell Mounjaro or Zepbound and do not describe our vials as an approved medicine.

Can a Philippine doctor prescribe tirzepatide for weight loss?

A licensed physician may prescribe the registered branded product off-label for weight management. That is their clinical decision, and the pharmacy will still require the prescription.

Where to buy. If, after reading the status above and speaking with your physician, a research-grade vial is what you want, Noki Labs stocks tirzepatide vials at PHP 2,400 (15 mg), 2,760 (20 mg), 3,600 (30 mg) and 4,200 (50 mg), each batch tested. See the tirzepatide Philippines pillar guide for research dosing ranges and storage, browse the weight-management collection, and pay by cash on delivery, GCash, Maya or bank transfer; free shipping covers cold-chain delivery over PHP 2,500.
