TL;DR
Tirzepatide vs semaglutide is the most common comparison Filipino readers ask us about, and the published record now answers it directly. In the only head-to-head obesity trial, SURMOUNT-5, tirzepatide produced a mean 20.2 percent weight reduction over 72 weeks against 13.7 percent for semaglutide 2.4 mg. In diabetes, SURPASS-2 showed larger HbA1c and weight reductions for every tirzepatide dose than for semaglutide 1 mg. Semaglutide still has the longer safety history and the wider branded availability, so "better" depends on what you weigh most.
The two molecules in one paragraph each
Semaglutide is a single-target GLP-1 receptor agonist. It slows stomach emptying, raises insulin release when glucose is high, and turns down hunger signalling in the brain. It is sold in the Philippines as Ozempic (diabetes), Wegovy (weight management) and Rybelsus (tablet), all by prescription, and we cover local pricing in our Ozempic and Wegovy Philippines guide.
Tirzepatide activates two receptors at once, GIP and GLP-1. The GIP component appears to add to appetite suppression and fat-tissue effects while blunting the nausea that GLP-1 alone tends to cause. It is sold as Mounjaro and Zepbound by prescription in markets that have registered it, and it is available in the Philippines from Noki Labs as research-grade vials for personal wellness use, a separate product category from the branded pens.
People searching "Ozempic tirzepatide" or "tirzepatide same as Ozempic" are usually surprised by that second point: they are different molecules from different companies, not a generic and a brand.
Tirzepatide vs semaglutide in the weight-loss trials
Each molecule against placebo
The two flagship placebo-controlled trials were run in similar populations, adults with obesity and no diabetes, but they are not identical in length, so read across them with care.
| Trial | Molecule and top dose | Duration | Mean weight change | Placebo |
|---|---|---|---|---|
| STEP 1 (NEJM 2021) | Semaglutide 2.4 mg weekly | 68 weeks | 14.9 percent | 2.4 percent |
| SURMOUNT-1 (NEJM 2022) | Tirzepatide 5, 10, 15 mg weekly | 72 weeks | 15.0, 19.5, 20.9 percent | 3.1 percent |
STEP 1 enrolled 1,961 participants and SURMOUNT-1 enrolled 2,539. In STEP 1, 86.4 percent of the semaglutide group lost at least 5 percent of body weight. In SURMOUNT-1, 85 to 91 percent of the tirzepatide groups did, depending on dose, and the 15 mg arm had a majority of participants losing 20 percent or more, a threshold that semaglutide rarely reached in STEP 1.
Head to head: SURMOUNT-5
Cross-trial comparisons carry bias, so the trial that matters most is SURMOUNT-5 (NEJM 2025), which randomised 751 adults with obesity to maximum tolerated tirzepatide or semaglutide 2.4 mg for 72 weeks. Tirzepatide came out at 20.2 percent mean loss (22.8 kg) versus 13.7 percent (15.0 kg). Waist circumference fell 18.4 cm versus 13.0 cm. More tirzepatide participants crossed every threshold from 10 to 25 percent loss. That is a difference of roughly a third more weight lost, in the same study, with the same measurement rules.
Tirzepatide vs semaglutide in type 2 diabetes: SURPASS-2
SURPASS-2 (NEJM 2021) compared the two molecules in 1,879 adults with type 2 diabetes over 40 weeks. Semaglutide was dosed at 1 mg, the Ozempic diabetes dose, not the 2.4 mg weight dose, which is a limitation to keep in mind.
| Arm | HbA1c change | Weight change |
|---|---|---|
| Tirzepatide 5 mg | minus 2.01 points | minus 7.6 kg |
| Tirzepatide 10 mg | minus 2.24 points | minus 9.3 kg |
| Tirzepatide 15 mg | minus 2.30 points | minus 11.2 kg |
| Semaglutide 1 mg | minus 1.86 points | minus 5.7 kg |
Every tirzepatide dose beat semaglutide on both endpoints, and the 15 mg arm lost nearly double the weight. For a Filipino reader whose main concern is glucose rather than the scale, the takeaway is that the dual agonist's advantage is not limited to weight.
Side effects and tolerability
Both molecules share the GLP-1 class profile: nausea, diarrhoea, constipation, reduced appetite and occasional vomiting, concentrated in the weeks after each dose step. Two data points separate them. In SURMOUNT-5, discontinuation because of gastrointestinal events was 2.7 percent on tirzepatide and 5.6 percent on semaglutide, even though tirzepatide produced more weight loss. In SURPASS-2, GI event rates were broadly similar across arms. The plausible explanation is the GIP action, which is thought to soften nausea.
Neither molecule should be used by anyone with a personal or family history of medullary thyroid carcinoma or MEN2, and both carry pancreatitis and gallbladder cautions. Our guide to tirzepatide side effects covers what to expect at each dose step and when to call a doctor.
Dosing patterns in the research
Semaglutide trials started at 0.25 mg weekly and stepped every four weeks through 0.5, 1, 1.7 and 2.4 mg. Tirzepatide trials started at 2.5 mg weekly and stepped by 2.5 mg every four weeks to a maintenance range of 5 to 15 mg. Those are the published ranges, not a recommendation; a licensed Philippine physician should set and adjust any dose, and the schedule detail lives in our tirzepatide dosage guide.
A practical difference for vial users: tirzepatide milligram amounts are larger, which makes drawing doses from a reconstituted vial easier to measure than the sub-milligram semaglutide steps.
Availability and cost in the Philippines
| Factor | Semaglutide (branded pens) | Tirzepatide (Noki Labs research-grade vials) |
|---|---|---|
| Philippine channel | Prescription only, licensed pharmacies; supply rationed in 2026 | Direct order, cold-chain courier nationwide |
| Observed price, as of September 2026 | Ozempic PHP 7,600 to 11,400 per pen; Wegovy PHP 10,000 to 35,000 per pen (market observations) | PHP 2,400 (15 mg), 2,760 (20 mg), 3,600 (30 mg), 4,200 (50 mg) |
| Verification | FDA registration number on the box | Batch certificate of analysis on our lab testing page |
| Payment | Cash or card at the counter | Cash on delivery, GCash, Maya, bank transfer |
The two columns are not the same kind of product, and the comparison is only fair if you keep that in mind. A registered pen carries pharmaceutical manufacturing controls; a research vial carries a lab report and a lower price. What research-grade means, and how it fits the Philippine rules, is explained in are peptides legal in the Philippines.
Which one wins for a Filipino reader
- Largest expected weight loss: tirzepatide, on every trial comparison available, including the direct one.
- Glucose control with type 2 diabetes: tirzepatide in SURPASS-2, though semaglutide has cardiovascular outcome data that tirzepatide is still accumulating.
- Longest safety record: semaglutide, which has been prescribed for diabetes since 2017 and has multi-year outcome trials.
- Easiest to buy through a pharmacy: semaglutide, when stock exists.
- Lowest cost per milligram: research-grade tirzepatide vials, at PHP 84 to 160 per mg at Noki Labs.
- Fewest GI dropouts: tirzepatide in SURMOUNT-5.
What you do with that list depends on your medical history, your budget and the supervision you can arrange. The week-by-week picture of what tirzepatide use looked like in the trials is in tirzepatide for weight loss.
FAQ
Is tirzepatide the same as Ozempic?
No. Ozempic is semaglutide, a GLP-1 agonist from Novo Nordisk. Tirzepatide is a GIP and GLP-1 dual agonist from Eli Lilly, sold as Mounjaro and Zepbound. Different molecules, different receptors.
Tirzepatide vs semaglutide: which caused more weight loss in a direct trial?
Tirzepatide. In SURMOUNT-5, mean loss was 20.2 percent versus 13.7 percent for semaglutide 2.4 mg over 72 weeks.
Does tirzepatide cause more nausea than semaglutide?
Trial data suggest the opposite or no difference. GI-related discontinuation was lower on tirzepatide in SURMOUNT-5, 2.7 percent versus 5.6 percent.
Can I switch from semaglutide to tirzepatide?
People do, but the switch should be planned with a physician, usually starting tirzepatide at the lowest research dose rather than matching the semaglutide milligram number.
Which is cheaper in the Philippines?
A Noki Labs tirzepatide vial starts at PHP 2,400 for 15 mg. Branded semaglutide pens were observed at PHP 7,600 and up in 2026, and they are a different, registered product category.
Where to buy. Noki Labs stocks research-grade tirzepatide vials in 15, 20, 30 and 50 mg sizes, each batch third-party tested with a certificate on our lab testing page.
